- When to call: considering transfer to hospital for urgent medical care, require expertise and unsure how to manage acute clinical change.
- What information should I have on hand when calling: Resident’s OHIP Number (gives GIM access to ConnectingOntario), resident’s vital signs, goals of care, past medical history and up-to-date medication list, secure email for sending consult notes, a phone number where you can be reached, name and location of ltc home.
